A Father that wants to Die

I have a Father that had Dementia and is in a local Assisted Living Facility. It is a really nice place that treats people well. My Dad is very weak and staggery. I am the only child and he had fallen at home numerous times. At least where he is at they can check on him. My problem is that he will not do anything to help himself or improve his quality of life. He will not take a bath, change clothes, take any of his medicine. I have to trick him into seeing his doctor, which that is a waste of time since he won't do anything the Doctor wants. I go to see him every day, but this is a real burdon on me. I can sometimes get him to take his medicine, but I can't do this every day and the Assisted Living Place will not do it. My Father is now beginning to show signes of involuntary movements, funny breathing and head shakeing. Who makes the choice about the Nursing Home ?  This is my last choice.

Each week I try to get him away from his environment, but he is getting very hard to handle. It will be very soon that I can't take him out without assistance. He has no balance and can't walk 50 feet ,even with a walker. I ruptured my intestines in January trying to pick him up and the Doctor has told me never to do that again. That is why the Assisted Living.

         Have any ideas ?
